Resources include templates, procedures, and other forms of guidance. Read below to see individual resources. To access all of them, download the applicable toolkit.
Clinical Resources
Forensic Clinical Resources are grouped by UST, NGRI, SVP, and Universal. Each item is tagged as one of three types:
- Templates: Orders, letters, forms, petitions, and clinical reports. These are editable starting points for filling in case-specific information.
- Procedures: Workflow guides for processes such as conditional release packet review, community provider monitoring, and petition handling. These are editable and reflect current IDHS practice for local adaptation.
- Illustrative items: IDHS reports are for reference and are not editable. The approved versions live in the IDHS document library on the internal intranet and are accessible only to authorized clinical staff.

- Templates: Editable starting points for filling in
case-specific information. - Procedures: Workflow guides for different court hearing types.
The original source materials were generously provided by the Honorable Judge Paul B. Novak of the 19th Judicial Circuit, drawn from his courtroom practice. These resources are not approved by the Supreme Court of Illinois and are not meant as binding authority—they are practical working tools that any county court can adapt to its own needs and preferences. Templates and procedures are provided as Word documents so they can be downloaded and modified for local use.
